Surest Health Insurance (UHC) — Has Anyone Had Your Procedure Covered?

Quick question for anyone on the Surest plan (the one that operates under UnitedHealthcare/UHC):

-Has anyone had it covered? -I'd really appreciate hearing about: How the approval/prior authorization process went (it any) -What actually got covered vs. what was denied. -Your real out-of-pocket costs -Any tips, surprises, or red flags

This plan is still pretty new to me and the details are tricky! Any experiences would be a huge help.
