Still thin on top, hair everywhere else

Am nearing 2 years on 2.5 twice daily oral minoxidil and have thick dark hear everywhere I didn't used to have hair except my scalp. Derm and primary haven't helped at all. Primary wanted to double dose of oral minox and add topical to scalp. Derm said I'm responding and it will just take time. Neither gave any other options and didn't think any other meds were appropriate. What should I ask for or look for elsewhere? I shampoo with rx ketoconazole and use as scalp treatment. I take rx vit d. Had iron infusions 2 years ago and have had great labs since.
