Toenail troubles

16yo male, ~175cm, ~50kg, 150mg Sertraline, don’t smoke, right big toe

I’m not very good at cutting my toenails, and it certainly shows in my right big toe. Near the base (where the nail meets the skin), the top layers(?) of nail have come off, leaving only a thin sheet of keratin over the skin. I don’t know whether just to leave it or to go to like a doctor or something, it doesn’t cause me too much pain other than when I pick at it (which I know I shouldn’t do).
