Can the items just spawn out of bounds or not spawn in at all? Can’t find a wrench anywhere

I swear I have been searching this one floor on the Lower Levels for over an hour and I can’t find this d*mn wrench ANYWHERE. I feel like I’m going insane and I’m starting to wonder if it’s bugged out and it’s spawned out of bounds or just didn’t spawn in at all.
