Experience appealing a grade?

Does anyone have experience appealing a grade? I’m an undergraduate in SPS, and my professor ghosted me for a month and ignored my disability accommodations. He graded everything on the very last day too. I don’t think his grade was fair, and ignoring my accommodations is unacceptable. Please let me know. Thank you.
