Form 821 after FQR

Seeking reassurance and clarity on another bump in the process.

I’m a first time applicant, I have representation (I learned my lesson with employer-based LTD claims). I’m in stage three and my medical determination was picked up by FQR on 03/19.

Received a letter today requesting I fill out the form SSA-821-BK (Burger King?). Now, the genius who sent the form failed to include an actual date of reference, so I’m waiting for them to call me back with that information. But in the meantime, I’m curious for any insight or lived experience with this particular branch of the path.

Necessary deets: my initial date of onset (the one I claimed in my application) is in December 2023, when I quit working full time because of my health conditions. However, I attempted to work a part time gig for about six weeks starting in February of 2024. I failed miserably and unfortunately lost a friend in my departure from the role.

So. My gut feeling is that they’re trying to determine the bare minimum of back pay then can give me, a hopeful spot for once, even knowing the data about success on initial applications.

Thanks for being a reassuring place for this whole process. There’s a lot happening in these conversations and it’s been more welcoming than when I call my lawyer.
