District Nurses

My Dad is supposed to have DN's go in twice a day to check on him; they're supposed to spend about 30 minutes with him each visit. He's recently been in hospital for a UTI and released with antibiotics. He's finished the antibiotics but still seems altered and having kidney pain. We've asked the DN's to dip his urine to see if it's clear, they ignored the first note we left for them and after having a long conversation with them they agreed to take one the following morning. My sister made sure the sample was done and left it in an obvious place for them. They called and said there was no sample so couldn't have it tested. Where do we go from here? My Dad's being neglected
