Bad hemorrhagic stroke? Speak to me docs!
71 year old Female 5ft 7in and 117lbs. My mom had a stroke. 4 days is hospital. Then to rehab. I was so uninformed at the time I didn’t know the right questions to ask etc. looking for some clarity now that the chaos has died down a bit. Was this really bad? There’s so much info about ischemic but hemorrhagic I’m having a harder time
She was on Eliquis for Afib. They said that combined with high BP was the cause. Even though her BP seemed to have been controlled. Now no more Eliquis.
She also had an angiogram which was unremarkable. And 2 CTs but here are the MRI results.
EXAM: MRI BRAIN WITH AND WITHOUT CONTRAST EXAM: MRV BRAIN WITHOUT AND WITH CONTRAST DATE: 47 INDICATION: ICH. COMPARISON: None.
TECHNIQUE: Multiplanar, multisequence MRI of the brain with and without contrast. MRV images of the brain were obtained utilizing a phase contrast technique. Subsequent time resolved postcontrast imaging was acquired. 3D spin and tumble reformats made at the scanner were submitted for interpretation along with the source images. IV contrast: Refer to MRI technologist documentation
FINDINGS: MRI BRAIN: MRI examination shows a large intraparenchymal hematoma in the left internal frontal lobe with heterogeneous hyper/hypointense areas on T1/T2 W sequences, and significant blooming on SWI, with surrounding vasogenic edema, resulting in remodeling of adjacent left frontal horn of lateral ventricle and minimal 5 mm rightward midline shifting. Minimal subarachnoid hemorrhage and small layering hemorrhage in the occipital horns of lateral ventricles. Scattered foci of chronic microhemorrhage noted in the inferior aspect of left frontal lobe and left temporal lobe. Multiple confluent foci of hyperintense T2/FLAIR signal in the supratentorial white matter, representing chronic microvascular ischemic changes. The intracranial arterial and venous structures demonstrate normal flow voids. The skull base, calvarium, and included facial bones are unremarkable. The paranasal sinuses are predominantly clear. Postcontrast images reveal no abnormal parenchymal or leptomeningeal enhancement. The vascular structures enhance uneventfully.
MRV BRAIN: Appropriate flow signal and enhancement is present in the deep cerebral veins and in the major dural venous sinuses.
IMPRESSION: * Large heterogeneous intraparenchymal hematoma in the left internal frontal lobe with surrounding vasogenic edema, resulting in remodeling of adjacent left frontal horn of lateral ventricle and minimal 5 mm rightward midline shifting. * Minimal subarachnoid hemorrhage and small layering hemorrhage in the occipital horns of lateral ventricles. * Scattered foci of chronic microhemorrhage in the inferior aspect of left frontal lobe and left temporal lobe. * Chronic microvascular ischemic changes in the supratentorial white matter. * Unremarkable MRV of the brain. Negative for cerebral venous thrombosis.
