Confused about application for Yunyu Kakunin-sho

Hello! I will be traveling to Japan at the end of May for about two weeks for the first time. I have OCD and need to bring my medication (Abilify and Propranolol specifically). I want to fill out the Yunyu Kakunin-sho just in case, but I’m having trouble navigating the application. Will I have to get a doctor’s note as part of the application?

On the other hand if they permit me to go in without an application do I just empty my prescription bottles so it only has two weeks worth of supply? Sorry for the dumb questions, I’m just very lost.
