For those on H1B on a surgical residency, how do you plan to fit in fellowship (if any) to be competitive for an academic job?

Some surgical residency programs I'm looking at are 5+ years (due to required research years), and given that I want to work in academic medicine, I will also need a fellowship which usually takes another 2 more years. Anyone who is or has been in the same spot, can you let me know how that is working out for you?
