PSA MIDDLE NAME AND MOTHER LAST NAME ERROR

So, both my mother maiden name at yung middle name ko ay same na may 1 letter clearical error. Some of my documents before is tama. Ang problema ko paano ako mag fa-file ng complain if my mother is not registered sa PSA(walang birth cert) and urgent na need ko sa school kasi graduating na ako. Isa pa is yung place of birth ng mother ko is malayo samin. Kaya di kami makapag registered sa kanya

Pwede ko ba ipa ayos yung middle name ko at error padin yung maiden name ng mother ko?
