29M Varicocele +Hydrocele + ED+ low testosterone+ 0.1 libido

Hi I am a masters student here in brandenburg Gemany near Berlin 29M, looking for a good doctor recommendations for my symptoms specially English speaking . I am facing a lot of stress due to my condition and dealing daily chores , I am dealing with this from past 3-4 years now ,used to do dr visits earlier too but then I stopped as no significant changes were visible.

I have pelvic floor tightness , dribbling urine and constipation , confirmed mini hydrocele in right testis and expected bilateral varicocele(never came in my scan) I can feel veins and all the symptoms one could have right side is more effected than the left one , no libido , low testosterone , numbness in whole region

Any guidance and help would be appreciated
