How common is a hospital in your colonies?

Title.

I have a few hundred hours in Rimworld and I realised I rarely if ever create an actual infirmary for my colonists. I usually have them thug it out in their bedrooms.

Sometimes I might create a small one with one or two beds, purely if I am dealing with a lot of operations simply for the sterile bonus but 90% of the time I find myself without one.

Is it something I should be getting into? Am I a big dumb smelly idiot for not making one sooner??
