Is anyone else seeing a massive drop in form completions lately?

I’ve been building something to try and fix the "dead" contact form problem by making them more interactive.

I just shared the project on U-Need to get some initial feedback, but I'm curious are people still finding success with standard forms, or is everything moving toward assessments and quizzes now?
