Avatar symbiote

Good evening, I need some help. I'm looking for an avatar and I need help finding one. I don't remember its name or creator, only its appearance. It's an avatar that blends the Prototype universe with a symbiote from the Spider-Man universe. The avatar is gray, eyeless, wears some kind of pants, and has numerous attacks and animations with blades, tentacles, etc. The creator used the Carnage model from Fortnite as a base.
