Internacional medical insurance

Hey, has anyone ever used the insurance internationally? My carrier is BCBS I called and they told me I can use it anywhere but is a little confusing about the claims. I’ll get a surgery outside of the US and the insurance mentioned something along the lines of guarantor or something but if someone has any information I would appreciate that.
