Judge denying Zoom/Continuance despite TBI diagnosis and Doctor’s "No Travel" order. 2,000 miles away and broke. Help? (PA)

TL;DR: I am a DV survivor with an ongoing case in PA, but I currently live in CA. I was just diagnosed with a suspected TBI (Traumatic Brain Injury) and have a doctor’s note explicitly barring "long travel." The judge is refusing a continuance and denying Zoom, ordering me to appear in person in 5 days. I am indigent (broke) and physically cannot fly. My PD is ghosting me. What are my options?
