Gore games without Guns

Im looking for games that are hyper brutal but without the power fantasy of something like doom. Some games turn into a point and click red mist simulator, and im looking for something slow and disturbing. I want to have to struggle to kill an enemy and not be completly overpowered if that makes sense.
