Has anyone else run into an issue w/ IBKR placing the same order twice?

I have credit spreads that I set a limit sell order on. Three times now it's hit that limit, bought back the short leg and sold the long only to see an opposite position.

Example
I had ten 101/103 bear calls on CRCL. I had a limit on this spread at .02 which it sold at for a decent profit. But then it also did what is show in the 2nd screen shot and shows 20 sells

For some reason It also did this. This is at least the 3rd time this has occurred for me. I eat a small loss and then commissions/fees on this. I'm not sure if this is something I'm fat fingering on occasion or what. Any input would be appreciated.
