41m, I experience symptoms of hyperacusis and a strange ear/hearing phenomenon.
I've dealt with hyperacusis for over a decade. Though irritating, it was something I could manage by carrying concert earplugs everyday in case I needed them. Around 5 years ago or so, I began experiencing something new and completely intolerable.
Whenever speaking to a person or watching videos of a person that possess a deep registered voice, my ears (maybe my eardrum?) pulse with each syllable they utter. I can feel the pulse, like my eardrum being tapped from the inside and hear it as well, sounding like VWHMP, that happens after the person starts talking. So if someone says, "Hey, come'ere and give me a hand," then a few hundredths of a second later one of my ears goes, "VWHMP, VWHMPVWHMP VWHMP VWHMP VWHMP VWHMP VWHMP," pulsing in time to each syllable spoken.
Sometimes it's my right ear and other times my left. By chance, I discovered that if my right ear is pulsing I can completely cover my left ear and listen through the right normally and vice versa. I imagine it's like certain sound waves travel through my eustachian tubes and uncomfortably bounce off the ossicles and ear drum in the opposite ear. The pulse behavior in my left ear changed about a year ago. Now when it acts up, instead of the VWHMP pulse, it still physically pulses but it's accompanied by a PSSH, like a small quick air burst through a valve.
The problem comes and goes. It might persist for hours, days, or weeks then disappear for weeks to months and abruptly start acting up again. When it's bad, and I mean by how sensitive the ears are to react, I can't even hold a conversation with my father. He's got a deep, resonant voice and I'll put in noise cancelling earbuds that have a good seal in my ears and then I can speak to him, but the past couple of days even that hasn't been enough at moments and I have to stop him and walk away.
I've seen a VA hospital ENT. They said things look fine. I tried a short round of antibiotics. I've tried several different nasal sprays I can't remember the names of. The one I'm on now is Budesonide. I don't know if it helps or not. I think it does a bit because it happens less frequently but it clearly hasn't eliminated the problem.
One last thing, I can sort of flex my jaw downward. I do that to pop/relieve pressure from altitude changes and it seems to help prevent the pulsing, but I can't walk around constantly closing and opening my mouth wide every time I hear somebody with a deep voice. I pretty much live with earbuds in. I even keep multiple pairs so I can rotate while the others charge. Oh, and I don't experience any other typical earaches.
