Claimant Reports

Hi All,

Had DVA reach out to me recently about an IL claim I had submitted (chondromalacia patella). They've sent me through a claimant report to complete and was wondering if anyone has received a document like this and what kind of detail they expect on the form.

I have significant knee issues. Some of the issues being the initial twinges I experienced with my knee occurred during service but I never sought help due to a mindset that if it isn't broken don't fix it.

After service I tore my meniscus playing rugby and have dislocated my knee a few times. I haven't submitted any paperwork for this kind of stuff before so I'm feeling a bit lost.

Any advice would be appreciated. Cheers.
