What are some of the most egregious mistakes/errors/oversights in a romantasy you've read?

I'm not talking shallow worldbuilding, I'm talking major goofs, like changing a character's backstory mid-novel, inconsistent physical descriptions (changing features), or forgetting subplots entirely.
