My doctor filled out the DTC form, but I don't agree with it. What can I do?

Hey guys. I have ADHD and GAD. I know the DTC form is about how these things impact you, and not the diagnosis itself.

My doctor recently completed her part of the form and I just checked it, and I'm honestly shocked. I know it's based on her medical opinion, but I really disagree with a lot or even most of her answers. It makes me wonder if maybe she doesn't really know or understand these things about me? I mean, it asks her questions about my daily mental functions, and I know its not patient opinion or response, but how would she know these things besides what I tell her? She doesn't really ask me or what not.

Few examples:

For every option in adaptive functioning, she wrote I have no limitations, but I believe that I do. I have a hard time adapting to change? I have a hard time going home to visit my family from university (even though its not so far away), because its a change in my routine, and its far too overwhelming not being able to do what I usually do. I can't change routine or else I cant do anything at all and I lose motivation. Its debilitating. Change feels unsafe to me because its unpredictable and i dont know what to expect, so because of this i avoid it.

She wrote that I have no limitations in concentration... direct quote because I annotated the document with my opinions and I dont want to retype it im sorry: "??? not unless i really like it and im hyper-focused, i cant focus at all. it takes me forever to get ready, to make food, to clean, to shower. any simple task i need to do (like getting somewhere, taking the bus) i have to deligate so much time to, to account for the things i might get caught in, because i try to makeup for my inability to do things with intense organization, but even that doesn't happen."

I guess you get it, this was almost every category. I just don't know what steps to take next. It's already submitted, and I guess it's less about them denying me, but more about me wondering if she doesn't know or understand that these are really hard things for me because I don't make it look like that, or if maybe they aren't actually hard at all in a real medical opinion, and it's all just in my head?

I hope i'm not overthinking this lol
