To medical professionals: how does a tenaculum actually work during insertion?

I've been considering getting an IUD and have heard a lot of horror stories about the insertion process, especially regarding the tenaculum.

Do the pointy ends actually pierce through the cervix to hold it or do they just pinch it externally?

I know there are tenaculums without pointy tips, but I'm asking mainly about the scary sharp looking ones.
