Heartbeat in ear

Hello. I am M 39. 180cm tall, 86 kg. No medications. Not smoker.

8 years ago I landed in the hospital for about 10 days due to an eye nerve inflamation that was present as persistent pain in the right eye. Since then several MRI with and without contrast of the cranium have been done, all but the first (showing the inflamation of the nerve) were without issues.

At that time started my issue, a sound of pulsating in my right ear. Especially, for example, if I go for a longer, faster run, for days after I will have a feeling of heaviness and slight pain around my right ear and also when everything is quiet I will feel the pulsating of my heartbeat like a pump in my ear. This makes me unsettled and I try to always keep my bpm around 160 or lower as to not trigger these feelings.

Any idea what is going on and should I be worried?
