Bosses have unexplained "+" bonuses

I'm trying to do T8 Lilith and she randomly has a +13 which makes it extremely hard.

Lair bosses also have random "+" bonuses that make them much more difficult. I don't have any War Plan nodes that would give + bonuses.

Any idea what's going on here?
