Is this a glitch or a hack??

there is a glitch where my controller randomly gets disconnected(it’s still connected through wire and it’s on but it doesn’t work) from my pc and my player just stands there doing nothing.

it happens during some games and other games move smoothly but it’s pissing my off.

if its a glitch then can you help me out on how to remove it?
