How to ensure continuity of care when a provider quits, and I am on disability leave?

I am struggling with an administrative issue, and I am also genuinely curious, as I don't understand why it's so complicated.

A very supportive LP put me on leave from work for a full year for anxiety and trauma. At the time they recommended, we decided it was the best course of action for long-term healing. It's been challenging, obviously, but I am incredibly proud of the progress I have made during the leave (I did talk therapy, sex therapy, conversational hypnosis, became a trauma-informed yoga teacher, took a life-changing somatic class, and learned about parts therapy - everything was so helpful, and I am so grateful I was able to focus on me).

My employer immediately accepted my LP leave request, so I didn't think twice. The LP who put me on leave at the time also filled out the State-required paperwork (CA EDD). It took months, but eventually the state asked for more information, which my LP provided, then the state asked for even more, every time denying my disability claim. Now my LP has left professional practice, and the office they used to work at won't take me on as a new patient because they are full, and they also won't fill out the paperwork since they don't know my history. Both my GP and even an in-network psychiatrist say they can't help because the leave has been requested by another provider.

Am I guessing there must be some fear of liability or lawsuits? I am thinking maybe my first LP gave me too much time, and no one else wants to back up a request for so much time. It's hard for me to regret accepting such a long leave because the progress I have made really does feel life-changing (yeah!), but this is a very challenging situation, and I would love not to lose my job out of it. Obviously, if someone has a recommendation for a solution, I will happily hear it, but I am also genuinely curious to understand why all the professionals I have contacted say they can't help. Is one year off completely ridiculous and excessive for CAD and Trauma? Is it risky to follow up on someone else's evaluation of a patient? I think understanding what is happening would help me accept it.

Thank you in advance!
