HELP ME on must have info on job order /estimate -- to Complete drain replacement under concrete slab for 1950s home

As title states, will seek estimates for complete drain replacement for 1 story 1950s home in Los Angeles, CA area. Currently have iron drains (I think 3" ). We have broken kitchen drain line and opting to replace all drains at once.

HELP I need is in what details to include in JOB ORDER/ESTIMATE must asks; ... (1) materials? (2) pipe widths? (3) concrete refill/fill excavated slab; (4) days to complete job; (5) hours of service ,.... anything else?

What are the green flags for better companies in the quoting process? (other than ensuring licensing)
