Severe perineal/vulvar and rectal pain after constipation and anal sex. P ossible pelvic floor spasm or fissure?

I’m having severe pelvic/perineal pain that started a few days ago and is getting worse.

Timeline: anal sex about 1 week prior, then constipation (likely menstrual-related), followed by painful bowel movements. I suffer with chronic constipation but usually have it under control except for before my period.

Symptoms:
Constant pressure/aching between vaginal opening and anus (perineal area)

Burning and tenderness extending into vulva and inner buttocks

Pain worse with bowel movements and sitting
“Bruised” deep pelvic feeling + mild itching

Bowel/other:
Passing stool and gas normally
No fever or systemic symptoms
Urine/vaginal swabs taken; external exam reportedly normal
On MiraLAX + stool softeners + ibuprofen + warm baths
I also had a little mucus and blood come out during last BM
Question: does this most fit pelvic floor spasm, fissure/internal irritation, or nerve-related pelvic pain? What would be the next step if exam is normal?
