How do I make an original universe + characters?

I desperately want to make my own universe to put characters in it, but I don't know how to. What tools are there to make worlds/characters and their info? How do I come up with initial ideas? What are some common pitfalls of OCs to avoid? I have no clue where to start at all, I don't have any inspiration for this currently which just sucks.
