orgasm and libido as a trans woman

How do some trans women cum? I see all these trans content creators making jokes about "drinking pineapple juice before a date" and the like. this doesnt make sense to me, i cant get out more than a drop of clear liquid? plus its never satisfying, and all medical sites simply say if you want to cum, to stop estrogen or stop spiro, which I'd kinda rather NOT do.
