IMPRESSION:
- There is a single unchanged punctate focus of presumed gliosis in the right frontal white matter. This appears greater
than expected for age. In the appropriate clinical context, this pattern is most consistent with white matter injury,
including that seen in traumatic brain injury. Clinical correlation is recommended. - Volumetric analysis demonstrates statistically significant atrophy involving the left primary sensory cortex. While not
specific, these findings may reflect sequela of prior traumatic brain injury in the appropriate clinical setting. - Diffusion tensor imaging analysis is notable for statistically significant abnormality involving 21 out of 24 measured
white matter bundles. This includes the corpus callosum, a structure characteristically involved in traumatic axonal
injury, with the overall pattern consistent with microstructural white matter disruption and a recognized imaging correlate
of axonal injury, including that seen in traumatic brain injury. - Taken together, the conventional MRI, volumetric analysis, and DTI findings demonstrate converging evidence of
both structural and microstructural white matter abnormalities. In the appropriate clinical context, this constellation of
findings is most consistent with sequelae of traumatic brain injury. Correlation with the patient's history, mechanism of
injury, and clinical examination is recommended.
