borderline 17-OHP + adrenal symptoms + pituitary findings — NCCAH or something else?

Hi everyone,

I have been dealing with some medical issues within the past 1.5 years. I thought I might post here to see if anyone has similar labs/experiences and has been diagnosed with NCCAH.

My symptoms started around September 2024 and have been getting worse since then:

very long/irregular cycles (81 days last cycle, currently day 49)
new facial/body hair growth (so much😭)
acne + oily skin (new for me)
fatigue
anxiety/mood changes
20 lb weight gain in about a month

My endocrinologist mentioned possible NCCAH, but my labs don’t feel super clear.
Key labs:
17-OH progesterone (follicular): 157 (range 15–70)
DHEA-S: 480 → 520 in one month
Prolactin: 87 → 45 (dropped within ~1.5 months, so not thought to be prolactinoma)
Testosterone, LH/FSH, thyroid, AMH, SHBG: all normal.

I also passed a DST test at 0.7 so she does not think I have Cushing’s disease. She also thinks my labs do not really align with PCOS.

Imaging / other findings:
2 small pituitary microadenomas (4mm & 6mm), but my doctor thinks they are incidental.
She said I would have a follow up MRI in 6 months to review them again.

Testing:
ACTH stimulation test is scheduled soon. She will
be testing multiple different hormones during this.

I’m wondering:

Has anyone here been diagnosed with NCCAH with borderline baseline 17-OHP like this, especially with normal testosterone and other hormones?

Or did your ACTH stim test end up being normal and it turned out to be something else?
Just trying to understand what’s actually typical in real cases while I wait. It’s so stressful 😭😭.
