If your SIBO keeps relapsing after rifaximin or herbals, this might be the missing piece nobody told you about.
The migrating motor complex (MMC) is a wave of contractions that sweeps through your small intestine every 90-120 minutes DURING FASTING. It physically pushes bacteria down toward the colon. Think of it as your gut's self-cleaning cycle.
Here's the problem: the MMC only works when you're NOT eating. Every time you eat — even a small snack, even a splash of milk in coffee — the MMC stops. It needs roughly 4-5 hours of fasting to complete a full cycle.
Now here's the part most GI doctors skip:
The #1 cause of MMC damage is food poisoning. When you get gastroenteritis (Campylobacter, Salmonella, etc.), your immune system makes antibodies against a toxin called CdtB. Unfortunately, CdtB looks structurally similar to vinculin — a protein in the nerve cells that CONTROL the MMC.
So your immune system accidentally damages your own gut nerves. MMC stops working properly. Bacteria aren't swept out. They overgrow. SIBO.
This is why antimicrobials clear it temporarily but it comes back — the nerves are still damaged.
The fix most people miss: PROKINETICS after treatment. Low-dose erythromycin (50mg at bedtime — used for motility, not as antibiotic), or natural options like Iberogast or ginger-based formulas. Plus strict meal spacing — 3 meals, no snacking, 4-5 hours between.
There's actually a blood test called IBS-Smart that measures anti-CdtB and anti-vinculin antibodies to confirm if this is your cause.
If you've relapsed more than once: it's almost certainly because nobody addressed the motility problem underneath.
Did anyone here get put on a prokinetic after their SIBO treatment? Curious how common this actually is.
