What to do after graduating in europe

Hi, I m just graduating in Belgium this year. Should I start residency in Belgium and study for the USMLE on the side, or should I go to the US and try to find a paid research position to better my CV while I study ?
I am a US citizen, I have done some research in the past but nothing that was worth being published.
