Anyone experience multiple updates with 221G k1 Manila

We submitted a strong joint sponsor on April 28 after my fiance got 221G. Today she got an email at 4:33pm Phillipine time from VFS global about her passport being returned but the CEAC website still says refused? Any insight? This would be 3rd 221g, if we got one.
