Re-applying for your ATT once it expires (FL)
For anyone in Florida who has to reapply for their NAPLEX ATT, I wanted to share the information I learned after spending 5+ hours dealing with MQA/Florida DOH customer services so hopefully other people don’t have to go through the same thing.
What I’ve learned:
In Florida, it appears you can only apply for your ATT online once, and your school usually uses that submission during your initial application process.
- If you need to reapply for an ATT yourself later on, you’ll likely need to complete the paper application instead of doing it online.

---->Pharmacist Licensure by Examination (U.S. Graduates)
- You have to print out and physically fill out the form then mail it to the address that is notified on the form this is due to them meeting physical signatures.
(Per what customer service told me)
- Payment of $295 needs to be sent as a cashier’s check or money order payable to the Department of Health Board of Pharmacy, and you can find their PO Box information on the Board of Pharmacy website under the "Contact Us" tab
- I also believe you do NOT have to redo the Form A, per the document, you can send an official transcript instead.
An official transcript should be sent directly to the board office from your educational program
The only things I’m still uncertain about are:
- Whether fingerprints need to be redone
(Personally, the CHAI website told me that my prints won't expire till 2030)

If anyone has recently gone through this process and can confirm those last details, please comment and help out.
The information online is extremely unclear, so I figured I’d share what I learned to hopefully save someone else a massive headache.
