Claude code going directly against the instructions given in the skill and it "knows" it

I have a unit testing skill that prohibits direct testing of internal services. Claude still writes tests for them and flushes my tokens down the drain.

I wanted to know the reasoning behind it, and it turns out it just straight-up went against the instruction for no good reason.
