Sheriff's Card info

Ok, let me start by just saying, I have tried already to get all of this information- scoured the LVMPD website, spent 15 minutes working my way through the endless phone tree.

I just want to know- do I need an appointment for the Sheriff's card? If so, how do I set that appointment? (I am going crazy trying to hunt down this info) And is this all occurring at 400 S. Martin Luther King Blvd? Is there a suite number?

I'm so sorry if this seems like dumb questions, but I can't for the life of me find this information anywhere.
