Re-doing SF86 form after failing?

Just curious for those who have received the SF background check form, how many have failed the background check, then been given a chance to do it again? I heard lots of people filled it out incorrectly, leading to them asking for a redo. Is this a potential reason for failing the check or would they notify the reason, and otherwise you failed for real?
