What exactly causes trims to fail?

If the damaged portion gets removed. What else is there that causes pain/catching/swelling?

My surgeon only recommended a trim if I’m having symptoms and I didn’t go through with it mostly because of everything I’ve read about still having pain and long term arthritis and chances of further surgeries
