Advise plesse
Hi I need advise please.
A patient received her medication which was IV cyclizine despite having no identification stickers in her notes, no wristband, and no name displayed on the board. I was the second checker. The patient was alert and had capacity we checked her details against her prescription, admission notes and her verbal confirmation.
My manager asked me to do a statement which I've done. However I've been asked to do a reflection but in the meantime I've spoken to the nurse who I second checked the medication for and she said that two nurses gave the patient cyclizine during the day shit, ON top of THAT a NNP and a nurse set up the patients TPN! Now I don't mean this to throw people under the bus but why us the night staff were targeted and asked to do statement and the other nurses havent?? We did call admissions to get stickers and were told they dont print them off for us anymore. We have had a new manager, my old manager wouldnt have approached the situation like this and im not saying im right as yes the patient shouldve had a wrist band on but they also knew that on the day shift when she was admitted do I even do this reflection or what?
Any advise plesse
