I(m/32) am stuck in a relationship with my long-term GF(F/35), and I'm doing everything in the relationship, how do I navigate this?

Me and my gf have been on and off since I was 15, we've currently been together for 5 years, I work, cook, keep track of the bills, etc, the only thing I've ever asked of her is the she helps clean since she says she can't work. We recently got evicted because our landlord saw how messy the place was but I was working 72+ hour weeks and literally just home long enough to cook for us and shower and sleep before heading back, we got a new place, I have a little more time off work now, and I'm trying my best to do everything, I think the flame is pretty much dead at this point, I want out, but I don't know how to do it, she has no family that will take her in, her mother is homeless and in another state and only talks to her when she wants money from me. She won't even go through the trouble of trying to think about looking for a job or helping keep the place clean. I took a pay cut just so I could try to do the cleaning myself on top of everything else. My question is, is it salvageable or just done? And how do I get out of it?

Sorry for the long text. I'm spiraling and dunno what else to do.
