An issue with equipping certain things

I picked up this weapon "dual bloodstained cleavers" that I would like to equip, but i can only see it in "preperations" and not in my inventory, wven when I try to equip it in the preparations menu it doesnt equip on my character. I've had this issue before, where an item would appear there but not in my inventory. How do I fix this?
