Accomodations

I'm going back to school as a 40yo. I failed the first two times miserably because I didn't realize I could apply for help with my learning disability. I'm very intelligent and understood the material I just couldn't get homework done or manage my time well enough. I didn't get help in elementary or highschool so I don't know what to even ask for as far as accommodations or what is even possible. Or if I need to be looking at like study groups or what? I need longer on tests but I also need help figuring out how to organize my day and someone to help me keep on track. What should I be looking for/asking about? For clarity I have ADHD autism and some extra memory problems from long COVID.
