I'm honestly impressed with how much weight the orforglipron pill can help you keep off

Discussion

I've been following the ATTAIN-MAINTAIN trial since the initial results came out, and the full publication in Nature Medicine is really worth a read. The study looked at people who had already lost a lot of weight on weekly injections of Wegovy (semaglutide) or Zepbound (tirzepatide), and then switched them to the once-daily oral pill orforglipron (Foundayo). The results? Those who switched from tirzepatide maintained about 74.7% of their prior weight reduction, compared to only 49.2% on placebo. The semaglutide cohort did even better, maintaining 79.3% of their prior weight loss after switching to the pill.

This is the first-of-its-kind Phase 3 trial to test an oral GLP-1 as a direct continuation therapy after injectables. So if you're someone who is terrified of long-term injection site fatigue, or you travel a lot and worry about refrigeration, this is a truly viable option. The drug is already on the market in the US for weight management with a cash price of $149/month, and the convenience is real. I'm not going to throw away my Mounjaro just yet, but knowing that there's a "step-down" oral option that actually works has taken a huge weight off my shoulders. It feels like there's finally a maintenance plan that isn't just staying on high-dose injections forever.
