Did your family Doctor read your Self-Report?

I am applying for mainly mental illness diagnoses, I also mention dealing with Autism Spectrum Disorder and ADHD. My family doctor will be the one who signs the forms for the application. She has 3 psychiatric assessments ( within the last 6 years ) from me in my medical file.

I only go to my family doctor once every 3-4 years for the common cold - so I feel like she doesn't know me much personally and my struggles... is it appropriate to ask her to read my Self-Report? I only rough drafted mine on my Notes App for now and it seems like it'll probably be 4-6 pages (it's a 30 minute appointment).

How many pages (was yours typed or not typed) was your Self-Report ?

Did your family Dr. read it during the ODSP sign-off appointment?

How long ^ was your appointment ? To me it seems like 30 minutes is a bit short...
