Please help me.

question

I've been dealing with severe front tooth pain for about 15 months and am looking for opinions from dentists or anyone who has seen something similar.
The pain started suddenly one morning after a week of clenching my teeth repeatedly while i was awake due to anxiety. I woke up with pain in my upper front teeth a week later, and since then it has never fully gone away. The pain is in my two upper front teeth and the gum area around them. The teeth feel tender to pressure, brushing can be uncomfortable, and talking causes burning or aching sensations.
Over the last 15 months I've seen multiple dentists and specialists. The teeth have consistently responded to cold testing, which has made providers question whether the nerves are actually dead. I've had X-rays, MRI imaging, blood work, and multiple evaluations.
Some findings so far:
MRI reportedly showed inflammation/swelling but no obvious fluid collection.

Blood work has been normal, including white blood cell counts.

I've been prescribed antibiotics in the past without improvement.

Steroids have also been tried.

Multiple medications for nerve pain have been tried, including pregabalin (Lyrica) up to 450 mg daily with no significant relief.

The pain has persisted despite all of this.

Recently I saw a surgeon/oral surgery provider who reviewed imaging and immediately noticed a very small area near the root of one of the front teeth. He told me that lesions and abscesses are often seen around tooth roots and said he wants additional imaging to investigate further. My understanding is that he saw something that could represent a lesion or abscess, but he was not certain from the current imaging and wants a scan for a better look.
My confusion is that:
The teeth still respond to cold testing.

The pain is in both front teeth.

This has been ongoing for 15 months.

There has been little to no response to antibiotics.

Previous imaging apparently did not lead anyone to diagnose an abscess.

My questions are:
Can a tooth with an abscess still respond to cold testing?

Can a very small lesion near a root cause significant pain?

Are there conditions that can mimic an abscess on imaging?

Has anyone seen a case where chronic front tooth pain for this long turned out to be a small lesion that was initially missed?

I understand nobody can diagnose me online, but after 15 months of pain I'm trying to understand what possibilities might fit this situation. Monday I will be going into the office and requesting the scan I had done that showed a lesion to be sent to my email and I will post it on here to show you guys and hopefully we can figure this out. it’s been 15 long months of brutal pain and I leave the house once a week for groceries. I am so depressed and had to even be put on an antidepressant because I was having dark thoughts from this pain never ending.

Thank you for reading. I do have other posts if anyone is interested in hearing more about what I’ve been going through. I’ll be posting the imaging Monday.
