Issue About Health Constantly Depleting

Alright so I finally got my Warlock build where I want it and I’m enjoying it but all the sudden my character has started to just lose health, like chunks ticking off when nothing is happening even when no enemies are near. It’s beyond frustrating. It doesn’t seem to be related to any skills, equipment, statuses, paragon board or anything else I can notice. Anyone have a clue what could be causing this?
